Requirements for Family Coverage
Employees may elect to cover legal dependents. Please submit the following applicable documents with the enrollment form. We are unable to enroll employees in coverage without these documents.
- Marriage Certificate, if applicable
- Birth certificates for dependent children, if applicable
- Legal Documents (i.e. Divorce Decree) stating requirement to provide dependent coverage, if applicable.
