Please complete the Farm Animal Registration Form and return it to the Health Department.
This confidential information helps the Animal Inspector Justine Grassey complete the State required Barn Book that many of you are familiar with.
The completed form can be:
- emailed to boardofhealth@holliston.k12.ma.us
- mailed to the Health Department, 703 Washington Street, Holliston, MA 01746
- dropped off at Town Hall (703 Washington St). There is a red drop box on the side of Town Hall with 24 hour access.
